HUMANA CHALLENGE SWEEPSTAKES

OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED BY LAW.

1. ENTRY PERIOD: Enter the Humana Challenge Sweepstakes (the “Sweepstakes”) between 12:01 a.m. Central Time (“CT”) on June 9, 2014 and 11:59 p.m. CT on September 30, 2014 (“Sweepstakes Period”).

2. ELIGIBILITY: Entrant must be 18 years of age or older and of the age of majority in their state of residence as of the start date of the Sweepstakes and must be a resident of and physically located in the fifty (50) United States (includes District of Columbia) at time of entry/winning and as of the start date of the Sweepstakes. Employees of Humana, Inc. (“Sponsor”), Charity Miles LLC and GMR Marketing LLC (“GMR”) are NOT eligible to enter or win; nor are the immediate family members (parent, spouse, child, sibling) of such employees as well as members of the immediate household of such employees, whether or not related. Sweepstakes is subject to all federal, state and local laws and regulations. Void where prohibited by law.

3. HOW TO ENTER : To enter, visit http://charitymiles.org/humanachallenge complete the Official Online Entry Form by providing your first and last name, daytime phone number, and e-mail address and check the designated box to accept the Official Rules, Charity Miles Privacy Policy, and Humana’s Privacy Policy, and click “Submit” during the Sweepstakes Period. Once you have completed this entry process, you will receive one (1) entry into the Sweepstakes. Limit one (1) entry per person during the Sweepstakes Period.

4. PRIZE AND APPROXIMATE RETAIL VALUE (“ARV”):

One (1) Grand Prize: A trip for the Grand Prize winner and one (1) guest to attend the Humana Challenge PGA TOUR Tournament in Palm Springs, California in January 2015 . Grand Prize includes: Round-trip, coach-class airfare for the Grand Prize winner and one (1) guest from a major airport near Grand Prize winner’s place of residence (which must be in the U.S.), five (5) nights/six (6) days hotel accommodations at the Renaissance Esmeralda Hotel ( in one (1) double-occupancy room, excluding room service, meals, and other incidentals),one (1) hour private golf lesson with a PGA professional for Grand Prize winner and guest (PGA professional to be selected at the discretion of the Sponsor), passes for Grand Prize winner and guest for four (4) days of the Humana Challenge, including one (1) day as Honorary Observers (passes selected at the discretion of the Sponsor), transportation to and from the Humana Challenge Tournament via shuttle service, behind the scenes tournament tour and VIP access to the clubhouse, media center, and driving range, invitation for Grand Prize winner and guest to the Humana Challenge Amateur Awards Party, autographed pieces of memorabilia signed by Humana sponsored PGA TOUR Pros (quantity of items and autographs will be selected at the discretion of the Sponsor), and $2,500 awarded in the form of a corporate check to the Grand Prize winner only, which is intended to be used for meals, rental car, and spending money.

ARV OF THE GRAND PRIZE IS $6,000

All federal, state local taxes and fees are the sole responsibility of the winner. Sponsor will comply with necessary tax form filing obligations.

Prizes are non-transferable and no substitution or cash equivalent is allowed except in Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize with one of the same approximate retail value. Grand Prize winner and guest must travel on the same itinerary as well as possess required travel or identification documents (e.g. valid government-issued photo ID) prior to departure. Grand Prize does not include meals, hotel room service; alcoholic beverages; merchandise, souvenirs, spa services; tips, gratuities; all incidental expenses; travel insurance; and all other costs and expenses not otherwise expressly mentioned herein. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein as being provided are the sole responsibility of the winner. All prize details are at the Sponsor’s sole discretion.

5. SELECTION AND NOTIFICATION OF GRAND PRIZE WINNER: The potential Grand Prize winner will be selected in a random drawing that is to take place on or about October 6, 2014 by GMR. Potential Grand Prize winner will then be notified via phone and/or e-mail on or about October 7, 2014 by GMR to the phone number/e-mail address associated with the potential Grand Prize winner’s Official Online Entry Form. The potential Grand Prize winner will be required to respond within seventy two (72) hours from the time of the first phone/e-mail notification to respond with their shipping address or the prize will be forfeited and an alternate winner selected, time permitting. The potential Grand Prize winner must complete, sign, and return (a) the presented Affidavit of Eligibility and a Release of Liability; (b) Publicity Release; (c) authorization form to conduct a criminal background check and to obtain required security clearance needed to attend the Humana Challenge Event, and (d) the required tax documents (the “Documents”) with seven (7) calendar days of delivery of the Documents, or the prize will be forfeited in its entirety. Should a potential Grand Prize winner fail to respond, fail to comply with these requirements, forfeit the prize, or be unable to travel during the dates specified by the Sponsor, or if security clearance is denied, Sponsor will declare the prize forfeited, and an alternate winner will be randomly selected as the Grand Prize winner, time and circumstances permitting. Return of any prize notification as undeliverable or failure to accept any prize within the specified time period may result in disqualification and forfeiture of prize. Winner’s guest will be required to complete a Publicity and Liability Release, a background authorization form to conduct a criminal background check and obtain required security clearance needed to attend the Humana Challenge Event. If a winner’s guest does not comply and/or is denied security clearance, that guest will be ineligible to attend as the winner’s guest, and another guest may be chosen, time permitting. Winner’s guest may be a minor only if Winner is the minor’s parent or legal guardian.

6. ODDS: Odds of winning a prize depend on the number of entries received during the Sweepstakes Period. Sponsor cannot predict the number of entries.

7. LIMITATION OF LIABILITY: This Sweepstakes is intended for viewing in the United States and is governed by applicable United States law. In the event of a dispute regarding the identity of the entrant, entries made on the Web will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry and the potential winner may be required to provide proof that he or she is the authorized holder of the identified e-mail account. If for any reason, the computerized entry portion of the program is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures or any other cause beyond the control of Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, Sponsor reserves the right at its sole discretion to disqualify any individual who tampers with the entry process and to cancel, terminate, modify or suspend the Sweepstakes. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries, including any error that may result in an erroneous appearance of qualification for a prize. Sponsor is not responsible for any problem or technical malfunction of any computer equipment or software that results in loss of entry. WARNING: ANY ATTEMPT BY ANY PERSON TO DELIBERATELY DAMAGE ANY COMPUTERIZED SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGE FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.

8. PUBLICITY RELEASE: The Grand Prize winner grants to Sponsor and Sponsor’s designees the right to the use of the Grand Prize winner name, picture, likeness, voice, video, biographical information and statements, at any time or times, for advertising, trade, publicity and promotional purposes, in all media now known or hereafter discovered, worldwide and on the Internet and World Wide Web, without notice, review or approval and without additional compensation, unless prohibited by law.

9. RELEASE; ADDITIONAL TERMS: Entrant agrees to accept and abide by the Official Rules of this Sweepstakes. All decisions by Sponsor regarding this are final. By entering, entrant agrees to hold harmless Humana, Charity Miles, GMR, PGA TOUR, and each of their respective parent companies, affiliates, promotional partners, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of the above organizations from any injury, loss or damage to person, including death or property, due in-whole or in-part, directly or indirectly, to the acceptance or use/misuse of a prize, participation in any Sweepstakes related activity or participation in the Sweepstakes. Sponsor is not responsible for any typographical or other error in the printing of this offer, administration of the Sweepstakes or in the announcement of prizes. In no event will more than the stated number of prizes be awarded. In the event Sponsor is prevented from continuing with this Sweepstakes, or the integrity and/or feasibility of the Sweepstakes is undermined by any event including, but not limited to fire, flood, epidemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state or local government law, order or regulation, order of any court or jurisdiction, or by other cause not reasonably within the Sponsor’s control (each a “Force Majeure” event or occurrence), Sponsor shall have the right, in its discretion, to abbreviate, modify, suspend, cancel or terminate the promotion. If Sponsor, in its sole discretion, elects to abbreviate the Sweepstakes as a result of a Force Majeure event, Sponsor shall award the prizes to winners selected from among all valid and eligible entries received up to the time of such Force Majeure event.

10. GOVERNING LAW: All disputes concerning the construction, interpretation, validity, effect, and enforceability of these Official Rules will be governed by the laws of the Commonwealth of Kentucky without regard to conflicts of laws provisions. Entrant (i) agrees that exclusive jurisdiction over any legal action arising out of or in connection with the Sweepstakes will be in state or federal courts located in or nearest to Jefferson County, Kentucky, and by entering consents to such jurisdiction and venue; (ii) consents to resolve any such claims individually and without resort to class action; and (iii) agrees that all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorney’s fees or punitive, incidental, consequential, or any damages other than actual out-of-pocket expenses.

11. WINNERS’ LIST : For the name of the Grand Prize winner, send a self-addressed, stamped envelope postmarked by January 9, 2015 and received by January 16, 2015 to: Humana Challenge Sweepstakes Winners’ List, 5000 South Towne Drive, New Berlin, WI 53151.

12. SPONSOR: This Sweepstakes is sponsored by Humana, Inc., 500 West Main Street, Louisville, KY 40202. The Sweepstakes is administered by GMR Marketing LLC, 5000 South Towne Drive, New Berlin, WI, 53151.
